Conflict of Interest & Confidentiality Forms


The safety of our constituents, volunteers and supporters is of paramount importance to us. The Alzheimer's Association maintains policies about conflicts of interest, harassment, background checks and confidentiality for volunteers and employees to avoid impropriety.

All program volunteers are asked to review these policies annually and disclose any conflicts of interest:
